Motivation:
This journal wasted our time. If they intended to defer to a single review about the appropriateness of a type of study design for the journal, this should actually be journal policy, rather than following the whims of a reviewer. Further, it is disappointing that they held this review in such high regard when the reviewer ignored all the revisions made in response to their initial comments.
